The HD Patient Navigator/Social Worker will guide patients and their families through our care continuum to identify barriers to treatment and assist with access to needed resources to decrease barriers. They will help in making an assessment of the patient’s needs, and work with individuals and their families to set goals and develop plans to realize those goals.
*Case management experience is preferred.
*Experience with neurological diseases is preferred.
Roles & Responsibilities include the following:
Assess patient situation, needs, environment, and support network
Develop plans to improve patient/family well-being
Work with patient and care partner to develop goals and an action plan
Facilitate access to community resources
Help patients/family work with government agencies to apply for and receive benefits
Follow-up with patients regarding their status and evaluate services
Help patients move through the complexities of the healthcare system by assisting with practical problem solving related to concrete needs such as lodging and transportation, and other services that decrease barriers to treatment.
Complete comprehensive assessments of the barriers to quality care that contribute to disparities and identify information, resource and emotional support needs of newly diagnosed and medically underserved patients and care partners.
Implement a follow-up plan for each constituent to ensure that services have met their needs.
Maintain accurate, confidential records documenting services provided in EMR (EPIC).
Assist in running the Northwestern HD support group.
Help plan and coordinate community educational and outreach events
Meet with vendors regarding services in the community
Develop written materials for educational and promotional programs
Work with NM media and printing to execute marketing materials
Distribute materials via new patient folders, conferences, symposia, community events
Oversee, develop, manage and facilitate event details including conference, support groups, symposia
Ability to work evenings and weekends at times when special events require it
Build and maintain relationships with key staff at NM, RIC and HDSA to build patient referral mechanisms and promote patient navigation services.
Collects patient and care partner stories for media and PDMDC request, as well for overall program marketing and promotion.
Help plan/coordinate Northwestern annual HD patient/family educational symposium, and represent Northwestern at other local HDSA sponsored events.
